Coast Guard standard procedure and 46 USC 63 require this type of report to be done for all marine casualties under Coast Guard authority. The investigation was conducted by the Coast Guard with the full cooperation of the National Transportation Safety Board.

“The most important thing to remember is that six people lost their lives in this tragedy and their families are forever changed. The investigation report and the final action memo will be used together to improve safety of life at sea,” said Capt. Lee Boone, chief, Coast Guard Investigations.
